I thought I'd post the final documents for the stereo boost I made for a mate of mine (who's surname gave rise to the pedal moniker and design). Thanks to everyone who helped steer me in the right direction in the other thread!! I learnt heaps during this process. It is a slight variation on this circuit, basically swapping the TL071 for a TL072 and doubling the circuit.

The idea was to have a stereo boost to go at the end of the pedal chain, after stereo modulation effects (reverb & delay).

(note the blue jumper to the right of R4 - or just skip it and take the output from there...)

I used a 4PDT footswitch. Because it's possible to use the pedal as a mono boost by just using one of the input-outputs, I wanted two LEDs to indicate which of the two channels was being used.
For this reason I made two independent Millennium Bypass circuits, each taking their ground from the RING of one of the (stereo) input Jacks.

The footswitch wiring looks like this:

I would change the input biasing to the noiseless type. 
Two 22k as the divider with two 1megs going to the opamps one more part a cap at the divider node

I would also increase the 1meg pulldowns to 2.2meg or 4.7meg you can go higher with a low DA input cap.  The input resistance as shown is just over 500K

I would also add a 100 to 1K resistor in series with the 10uf cap to output volume controls.  This is to help keep the opamps stable.  You can also add a series input resistor.

Maybe this will help. 
I posted this at the forum in the past. 
Textbook type circuit omit the guitar and cable sim if you build it(left of input and the x). 
It has an optional low pass filter at the output.
