Madbean UPROAR BUZZ Help!! SOLVED

Hey All,
So I built this Mad bean Uproar Pedal a few days back and it was all working very well, I shipped it to someone and when he plugged it in, all he could hear was a buzz, whether the pedal was on or off, which is weird, since just before putting it into the box to ship to him, I had tested it and it worked just fine. I am now thinking of what may have caused the issue, I have got the pedal with me and I tested it and it does the same, buzz when on and off, swapped the OPAMPS, still no go, just wondering if this could be the 3PDT footswitch? any suggestions are greatly appreciated please.
No loose wires, no shorts, no bad contacts....

Would highly appreciate your inputs please...
