Looking for A/B A+B bypass loop schematic

Hey guys,
I've been trying to hunt down a schematic for a 2 channel loop with
A/B switching, A+B and a bypass and can't seem to track one down.
I was looking into the BYOC A/B loop but really want the added A+B.
I saw this guy (attached image) in some searches and would love to make
Something exactly like it. Also if anyone has any leads on what
Size that enclosure is/where I might find one, it would be most appreciated .

Thanks for the schematic, this is a bit more complex than what I'm looking for however.
I'm certain that I can some how modify this one (below) with anouther switch for the A+B but I'm just Not 100% sure how.

This is BYOC's 2 loop a/b with a bypass I just need to figure out how to add that A+B
