Ross Phaser layout

Hey guys, I'm working on a Ross style phaser and I'm having some issues. Any help would be appreciated.

I've gotten some PCB's made up, built one and am having some problems. This is the OTA Ross phaser with an added switch to choose between 4/6/8 stages.

It seems the LFO isn't working at all. There is a vibrato switch (SPDT on schematic)- when this is switched to have the dry signal on I'm getting that but no phase. When I switch it to just the phase signal I get nothing. It's like the circuit doesn't want to phase at all.

I've double checked values and connections (I think I'm going mad) but anyone that has any advice on this or has built it I'd love to know if you've come across any issues. I have fed each of the 4 stages with a seperate output from the LFO labeled 'A' and 'C'. I'm not really sure if I should've done this or just used one output for all 8 stages.

Any advice would be greatly appreciated.
