Stereo guitar hum

With my new amp setup, I get a good deal of hum using my stereo output on my guitar.  I assume it's grounding issues - since each pickup's hot side goes to one wire in the stereo cable, but the grounds both go to the same ground, and then everything just gets split by a y adaptor or simple three-jacks-nothing-else box.   I'm assuming using audio transformers (I've got a few of the mouser 42TM018's kicking around) to isolate the grounds would help this issue - but I'm kind of flaky here and not sure on the best way to connect them and whether or not I'll need additional pre-amp in the box.  

I was thinking my plan maybe to just use the splitter circuit on Geofex (here: http://geofex.com/FX_images/splitter.gif ) and double the circuit - one for each stereo wire, leaving out the direct output, and only connecting one transformer to output section from each one of these.  (although - driving four amps . . . )  But is this more complex than this really needs to be, or will it work?  Am I just lost?  I'm usually just lost.
