what size, how many, double sided, plated throughs, solder mask, silkscreen?

Lots of parameters to see who's best.

I ONLY use fabrication houses that recycle their etchant and dispose of all waste properly. I won't use cheap board houses in Asia or Eastern Europe for this reason.

With most places there is a break at some quantity and PCB making is much about setup costs. For all my small run projects I've been using ExpressPCB amd the results have been wonderful every time. They have a quantity break at 50 boards so use that for figuring short runs if you can. For prototypes they do a 3.8 x 2.5 inch double sided board with plated throughs. You get three boards for $59 (outside California, or $63 in California).  8)

In response to the questions in the forum - PCB Layout for Musical Effects is available from The Book Patch. Search "PCB Layout" and it ought to appear.
