help a newbie debug a phase 90?

Greetings .. and thanks in advance for all the helpfull info I found here in the past.

I did my best to follow the "when it doesn't work." guide .. but please feel free to correct me if I've made mistakes .

as far as I know this is a stock unit .. it has never worked for me .. it powers on/off as it should and allows the signal to pass through it .. it does color the sound but has zero swooshing or phasing.
