R.O.G. Fetzer Valve: can't get it to bypass.

I hate to admit it, but I'm still at a fairly paint-by-numbers phase in my pedal building.  I just finished a Runoff Groove Fetzer Valve (Fenderiffic!).  It works great, but I can't get it to bypass properly.   I used the template from the General Guitar Gadgets OSQ as the basis for the wiring of my 3PDT switch, which is obviously not right.  Can anyone throw me a workable diagram or "gut shot" photo of a working Fetzer?
