Muffer build not working?

Hey guys this is my first build and am have some trouble getting it up and running

any suggestions are welcome.

I have built a Muffer circuit fro AMZ.
I used the multi purpose board that can be purchased from the web site.

The only thing I did different was add a LED as an indicator I also replaced R6 with a Red LED.

I first pluged it in and the LED would turn on when i didnt have a plug in the input jack but fixed that I think the terminals were shorting on the box.

Then I had the pedal working fine for about two minuets.

It then stopped working and I cant get it going again. everything looks like it is connected even if it is messy.

There is intermittent  power getting to the indicator LED and will dimly go on when the switch is pressed but does not go on perfectly.

I am completely bamboozled why it worked perfectly then stopped.
