Buffer + Headphone Amp (how to?)

Hello!

New here, registered to ask these questions:

1. I'm building a box that will serve as my monitor solution for violin. The connection will be violin - box - sound engineer, and then a headphone jack on the top, with a volume knob. I've already built a buffer for this purpose, but I'm kinda stuck at how to connect a headphone amp to the system? Can I just "steal" the signal from the chain somehow?

Meaning: can I build the buffer by itself, and then the headphone amp by itself....and then connect them somehow, or do I have to build them into the same board to make it work?

2. Can I build a headphone amplifier that will have a strong enough signal for on-stage purposes? I've been looking at this: http://www.minidisc.org/schem.gif

Yes.
Yes.
How would I go about taking the signal from the chain?

The OUTPUT needs to have a clean signal from the buffer part. Would I just solder two sets of cables from each input jack, one set going to the buffer (which then goes to OUTPUT) and the other set one going to the headphone amp (which then goes to the headphone jack)?

Would it be best to feed the headphone amp with the buffered signal, or the clean signal?
