Just wanted to update this in case anyone stumbles upon it with the same problem. Turned out to be a bad LM358. I spent a lot of unnecessary time, because when I replaced the LM358 I thought it didn't fix it. It didn't fix it because I had it in the socket backwards. So often it's something simple you've overlooked. Thanks for everyone who helped!
