Need help finding 1/4" jack for Boss pedal

I'm replacing  a jack on a Boss DD-6 and am having trouble finding the same part.  It has 3 solder terminals, one of which is on the outside that grounds to the box when installed.

Which jack is it? And how many wires are going to it?
