Jack Orman's "Mosfet Boost" 3pdt wiring

I purchased Jack Orman's "Mosfet Boost" pcb.The literature that came with the pcb only show's how to wire it with a dpdt switch.Does anybody have a diagram for 3pdt wiring for it?I just need the led to know when it's on.

On the support page under the section labeled "Wiring Diagrams"  is a link to a basic stompbox wiring diagram that includes a 3PDT.
