soldering tip falling out of pencil?

has anyone ever had a problem with their soldering tip falling out of the end of the pencil?
i have a wes51 and just got a new tip maybe 6-8 months ago and just today it fell out of the pencil.  i tightened it and it just won't stay in!  it's still tight enough to make a few joints, but i can feel it wiggling around in there when i touch it to something.  i ordered a new tip to see if that will fix the problem, but has anyone else ever had this happen to them?

I wonder if there is some oxidation, etc in the screw slot keeping you from being able to tighten the screw all of the way.  You might try removing the screw and cleaning the threads/screw or possibly replacing the screw.

All that heating and cooling.expansion and contraction along with
using a wet sponge will do that...
