Please help with this obscure capacitor code

Hi everybody - Picked up a big bag of silver mica caps at a hamfest yesterday and have had a hard time figuring the coding on it.
The brand is "CDM", which is printed on left side vertically. Top row has "CM05FD" and bottom row is "151F03". Below that printed vertically on right side is "R".

Well, I figured out the "151" must be 150pf, but I can't figure voltage or tolerance. Mainly interested in knowing voltage rating now.
Any help/info on this cap would be much appreciated. Thanks!!

seems to be rated 500v and +/-1% tolerance.
