PNP Fuzz working with battery but not adapter

Hi there.
This isn't actually my own build but I have done some pedals in the past. The pedal is a Carls Custom Bender/Fuzz Face.
The pedal sounds really good but it won't work with a power adapter. Works fine with the battery. I haven't been able to get any info from Carl's.
I'm using the Voodoo Labs Pedal Power Plus & the required reverse polarity cable. I've used this power supply for other PNP pedals with no problem.

I can't see any wiring issues but I'm kind of a novice so any help from the forum would be appreciated.

It's late here and I might be out of it, but it looks to me like if the pedal is wired like the image you linked, you shouldn't need a reverse polarity cable. With a standard polarity cable connected to a power supply like the Pedal Power Plus, the center pin is negative and the outside ring is positive. The image you linked to shows center pin to "board -9V" and battery negative, and outside ring to ground (positive). If it were my pedal, I'd try it with a standard polarity cable. But I am not going to tell you to do that, lest I'm wrong and you fry your pedal.  ;D  How sure are you that the pedal is wired like the image you posted? Did you trace the wiring and confirm that?
