Music PCB Uglyface Build - Not Working

1.What does it do, not do, and sound like?
Signal works bypassed.  Once the effect is "engaged" there's no sound and no LED; you can hear very faint scratchy sounds when you turn the potentiometers.
2.Name of the circuit = uglyface.
3.Source of the circuit (URL of schematic or project) = musicpcb.com, used pdf supplied on their website to wire everything up
4.Any modifications to the circuit? No.
5.Any parts substitutions? No.
6.Positive ground to negative ground conversion? I assumed it's negative ground.  Using a One Spot to power it.
7.  I don't have a multimeter on hand, just posting this to see if there's any obvious errors connected with what's going on.
I can check out the voltages later this week if needed.

I covered the back of the PCB in electrical tape to avoid short circuiting.

Hey sidewalk, it looks to me there is something about the switch wiring that doesn't look right.
Did the circuit work before you wired up the switch? I've taken to testing circuits and making sure they work before doing anything else.
That way you've already debugged the circuit. Then if it doesn't work after the switch/jacks etc. then you only have to deal with them.

That's the layout I used. I did not test the circuit beforehand and my input jack is mono.

Well it all looks right to me. Next step is to post voltages according to the Debugging thread.
Some one will notice something there quite quickly, if they're off.
