Axis Fuzz from fuzz central, anyone has Rev.1 layout?

I got a pcb from a friend who etched it years ago, and I can't seem to find a layout for it. On fuzzcentral's site there is a rev.3 going from 2005 but I can't find anything on the internet of rev.1
Maybe someone has it in their archived layouts and BOMs?

Would be much appreciated. Thanks.
