What kind of jacks and switch are these

I noticed this picture on another topic. The arrangement of the switch between the two jacks is nifty. This could be useful with some tight projects. I'm assuming the box is a 1590B size. What type of switch and jacks were sued here?

I just designed a new fuzz circuit! It almost sounds a little different than the last fifty fuzz circuits I designed! ;)

Regarding those stomp switches, have read of people having issues with Tayda's 3pdt foot switches so was wondering about the quality of those small bodied DPDT's. Looking for something small like those (or the Alphas at Small Bear) to use in conjunction with relays or electronic switching.

I can't say for other people, but I've never had an issue with tayda footswitches...yet. Their jacks leave something to be desired though.
