Can this circuit run a small speaker? So I can use this circuit as a small amplifier. Its basically a preamp into a valvecaster into of a ruby amp?If so, do I need changes on the output side?

You've answered your own question there - the Ruby's an amp. The LM386 was designed to drive speakers, we just abuse them when we're using them solely as distortion generators in a stompbox. I used to run one into a 4x12, volume-wise you had to raise your voice to talk over it.
