Pickup Selection Presets..not really a pedal thing but..

Been thinking of reinventing the pickup selector for my guitar....2 humbuckers, means 4 different outputs (2 HB outs and 2 coil tapped outputs)....the idea is to take these 4 signal choices into,say 5  DIP switches (for 5 presets), these DIP switches can be mounted in some recessed area of the guitar, or even the front (have a Strat so not much drilling required).
What I'd like, I guess, is some sort of pickup selector switch that is 4 pole (one for each pickup out) and 5 throw (one for each DIP preset)...rotary would probably be the easiest way to do it, but can this 4P5T be made with CMOS, or slide switches,or something else?

The old electra guitars had this 5 way switch on it for selecting various combinations of pickups. The 5 way selector allowed for selecting both HB in series and in phase, bass only, both Hb in parallel and in phase, treble only, both in series and out of phase.

I have an X310 with this 5 way on it and it is fantastic. So many tones available.
