Phaser problem

Hi everyone im new here and have a question .
I built 5 pedals that is working (dist. Delay tremolo amp simulator etc.) nowadays im trying to build a phaser . I built 2 of tonepad projects (small stone , rossphaser)  two of the boards had worked. I played with the board with pleasure then i heard some kind of cracking noise and then nothing the signal lost. This happened for two of the boards. Where am i making wrong. Can you help me ?
