Boss DS-1 Keeley Mod Issue. Loud Hum

Hey y'all.

Doing my second Keeley ds-1 mod for a friend. First one went smoothly with no issues, but just so happen to have issues when it's a friend's pedal I'm working on. All I get his a loud hum when the pedal is turned off or on. The power LED cuts on, but the seeing eye LED only flashes once when the input cable is plugged into the pedal. Here are some photos:




I had a cap bridging D4/D5 but removed it to check the solder joints. Still the same issue. Help!!

Check the wires going to the jacks, maybe one pulled loose.

Wires to jacks seem to be secured to the jacks themselves and the board.
