Keeley Blues Driver mods... I've lost it

There used to be a page out there in the World Wide Wonder Web but I cannot find it anymore. I did this mod to a friends Blues Driver which has since been stolen. He's replaced it with a new BD and would like the mod done again. I need some help please.

I don't know if it's the complete mod or if it is 100% acurate, but it should be a good start.
