Black ice cube tremolo module

I need the connection diagram for a tremolo module I bought years ago. I believe it is made by Carl Cordover Co. I have had this since the 70's and all I know about it is that it uses a 25k potentiometer ( because I packed that in with the module some time ago.) It came in a bubble pack on a hang card and that had the directions on the back. Of course that has gone missing!!!
Can anyone help?

I wonder if its one of those things that plugs into your amp's spring reverb wires, instead of the reverb tank. :?:

No. It is a inexpensive kit module that is encapsulated in black epoxy. There are six leads coming out of it. If I was to guess, probably in, out, ground, V+ and the other two connect to the 25k pot. The question is which is which?
There were others in this series. Audio amplifier, Burglar alarm latching relay, etc. If anyone has the diagram in their junk box, that would be great.
