Hookups For Orange Squeezer Clone

[H5N1]

I used Mark M's layout for the Dan Armstrong Orange Squeezer, but can't figure out the hookups for the in, out, led, etc.

I'd also like to put an external power jack on there. 
Can someone walk me through the connections or put up a quick pic?

  Ground is the common reference for Input/Output, both sleeves must have ground if you're using jacks yet.
  The output is from the wiper of the volume pot, and that goes to the output jack [through a bypass switch? if you have one yet].
  Input is marked and comes from the source, or input jack tip if you have one yet..[and through the bypass switch if you have one.
