AMZ mini booster vero layout

Hi,a few years back i was without a printer and hand drawned the above.Now i want to build it but it looks like i made at least one mistake.
Trouble is i can`t find the original back and i don`t remember who made it in the first place.
There was a guy who s name i just can`t remember that made an awfull lot of vero layouts but suddenly vanished from the forum.
Someone else(i think it was indyguitarist)hosted the layouts at his site for a while.
Does anybody have an idea what i am talking about and where i can find these layouts?
