Any ideas on how to make one? i would like to put it with a od/dist circuit on a momentary switch, which i think would be pretty cool. Does anyone know how to make this?

did a little searching and found this from WGTP Sonic Assault's album:

i get it except for a few questions:
1. do i send 9v directly to pin 8? and then where it says 4.5V i should get 4.5v?
2. What kind of IC would work?
3. are pins 5,6,7 just chillin' and unconnected? second time ive tried to read a schematic :(

1. Yes -- that is the v+ supply.

1a. Create a voltage divider to get 4.5 volts.  This is frequently noted as "Vref" on schematics.   In this case, you will want to use two equal-valued resistors (10K is a good choice).
The two resistors are connected in series between V+ and Ground.  The 4.5v reference value is at the junction of the two resistors.

2. The pinout tells me that this circuit was designed to use a dual op-amp (based on the pin assignments).  Something like a TL-072 would fit the bill for this.

3. 5,6,and 7 are unused in this circuit and can just chill for the time being.  (You could use them for a future enhancement such as an active tone control, etc.

Thanks for the help! now to make a circuit to incorporate this with...

That IS the circuit though. The only other things you'd need is the power supply (just 'borrow' from the Tubescreamer power supply), possibly changing that 47 ohm resistor ground to a pot and maybe increasing the value of the input cap.
