Stereo Pedal output mixer ..help

so i want to use a stereo volume pedal to switch between fx ...  say like:  heel  = octave , toe = autowah .... doing so i currently have to run two cables from the stereo volume pedal into the amp (dual input amp) .. what i need assistance with is building a mix box for my pedal board so i can run only one cable from pedal board to amp ... my problem is i built a 2 1/4 jack to 1 1/4 jack device but  it gives off awful feedback sine noise , as if the two signal are feeding into each other, or back into the pedal or something ..  ... the mixer box i made is simple  passive box ..  i see there are commercial 4 channel mix boxes that would fit a pedal board , but if theyre passive im sure it would produce same problems/results..
so i guess im asking for advice on making an active  2 to 1 1/4 jack mix box ... if possible .... something to fit a pedal board... o'r am i going to have to use 2 to 1 stereo cable ... which i was trying to avoid just to reduce need of 'special' cords ...
