Sorry, ignore my post about the trimpot.  I didn't see the 2 10k's going to +9v and the voltage divider.  My bad. 

No problem. I was just confused about what you meant.

So the arrow pointing to Vb is already 4.5v, because the 10k resistor going to 9v, and then the 10k going to ground? 

Yes. You may want to read up on generating reference voltages at GEO; its in "Circuit Sweepings".

I came up with a layout based on the TL074 and R.G.'s schematic, thanks for all the help guys.

The pad on the bottom left corner that isn't labeled (parallel to the word P1) is P3.

Pin 1 of IC is top right corner of the IC, and I made as if I'm looking at the layout from copper side. 
R5 is mounted on the bottom of the board.
