1590NS 4 Knob + LED Drilling Template?

Hey All!

I was wondering if anyone has a drilling template for a 1590NS box set up for 4 knobs, LED, as well as all the other goodies (input, output etc..)  I was trying to buy one pre-drilled from Pedal Parts Plus but apparently they can't fulfill this order without a template.  I don't have an easy way of doing this on a computer so I thought I might take a shot and see if anyone else has been in this (what I would think) common situation.

p.s.  I'm looking to orient the pedal with the long side vertical i.e.
