1/4" board jack: identify the manufacturer?

I am trying to find the replacement for the board-mount, 1/4" jack featured in the following pictures. There are no manuf. # or ID. If anyone has any leads that would be great -thanks.

Have you checked out the fender 9 pin jacks you might be able to make it work.

Looks like the same jacks used in the Russian Big Muffs

Definitely Fenders.  A lot of guitar parts sites have them.  Ebay.  Buy two.  Like the one pictured, they love to break.
