Phase 90 debug

So, i built the Tonepad phase 90 ( http://www.tonepad.com/getFile.asp?id=42 )
I haven't substituted any parts (I've used a log pot and wired it as a rev. log but that can't really matter).
I bought a set of matched FET's from Banzai and haven't got the right op amp to check the bias.
Problem? It sounds like a tremolo. Sound clip is at lowest rate and then at about half way up.

What worries me is the varying voltages at Q5. I haven't seen them in any other Phase90 debug.

Obvious, but do you have the correct pinout for the 4125?  And checked for solder bridges (especially near Q5 emitter on tonepad layout, 150k resistor off of pin 5 of IC1, or between E & B on Q5)?  I'd check there scrupulously first... also might wanna look at pins 5 & 6 of IC1, IIRC you should have swing there.
