Concentric Pot Question

I need some help finding a replacement pot for the front panel I/O Levels on my Lexicon MPX G2.  Lexicon is out of them, and can't help.

The pot in question is a 10K dual concentric pot, with Input level controlled by the inside knob, and output controlled by the outside knob.

What I don't understand, is why it has 3 rows of lugs, instead of the usual two rows that I'm used to seeing on dual pots.   Each row has 3 lugs.

Is this basically 3 10K pots in one? 

The spot on the PCB that  I removed the pot from, looks like this:

Can anyone tell me the name of this type of pot, so I can find a replacement?  If anyone can point me towards a suitable replacement, that would be greatly appreciated.

Maybe it's actually a dual volume and single level pot?
3 pots actually. One shaft/knob controls the dual section
and the other controls the single section.

Actually, I think you're right.  When removing the pot, I did notice that one of the knobs controlled the front two wipers, and the other controlled the rear single wiper.  

That would be a single pot for the input, and a dual pot for the output.  All wrapped up in one pot. 

Any idea what this type of pot is called, or where to find such a pot?

A little further research, and I've found this pot, which looks like a 4-channel version of the pot that I need:

4-channels Miniature Dual Concentric Shafts
10Kx4 Potentiometer (LINEAR Taper)
Body = 12mm x 10mm x 16mm
Resistive element: Carbon
Outer Shaft: Dia=3mm (D-section)  Inner Shaft: Dia=6mm Split
Shaft Length = 25mm (inc. Bush)
Mounting Bush: M6 L7
