OCD problem

I just got an OCD from this guy and someone has seriously butchered it.  It's a V4. I have a couple of questions and hope you guys can help. D3 is gone...need to know what to replace it with.  Also, the power wiring is completely shot.  Which pads on the board are for the 9v and ground?
