to figure out how to wire any jack, just use a dmm to see what connects to what...for instance, touch one probe of  the dmm to the inside of the jack, then touch the other to each of the lugs. the one that shows 0 resistance is the casing...etc
always think outside the box

Jacks often switch in a way that is not immediatly obvious.
Inserting a plug into a jack can cause many switching assignments to occur, there are many types of jacks, two types of plugs [stereo/mono].
Following the traces [look into the sandwich layers of stacked traces/insulators from the side, turn the pot to find the lug-tab-bend in the copper trace].
For each connection[s verify that the visual connection is indeed correct and measure what other connections are made/broken if a plug is inserted, repeat with plug removed.
  Some of the 'tricks':
Ring/Sleeve of stereo jack become common when an inserted mono plug sleeve connects them, often seen/used to disoconnect power/ground in effects, this is the trick used on effects that say: "to save battery, remove the plug from the effect input jack.
Tip of jack is grounded until a plug is inserted, this is often used on an input, it keeps the signal input grounded until something is plugged in.

Convention creates following, following creates convention.
