If it happens intermittently, and is fixed by slapping, then it's most likely just a simple short or disconnect in the box wiring.

The easiest way to track down the problem is to reproduce it.  Sometimes a little tenderness works better than slapping (sorry, couldn't resist).  When it cuts out again, instead of slapping the box, gently wiggle all the plugs in their jacks.  Connect and disconnect the plugs.  Also try wiggling the pots and switches.  It's best if the wiggling doesn't cure the problem, because that makes it easier to confirm the cause in the next step.  Carefully open the box (you want to still have the problem at this point).  Before moving anything, make sure the problem is still there - sometimes opening the box cures the problem.  Look into the box to see if you can spot where any wire or terminal is in contact with it or might be pressed into it by the lid.  Then try moving the parts inside - and notice whether moving anything in particular makes a difference.  If it's still in a non-working state, then start measuring with your meter.

Here are some things to check:
1. Verify the power connection - is the battery/power making it to the board?
2. Make sure there's no continuity between signal and ground (on both input and output sides) - are any terminals on the jacks, pots, and switches in contact with the enclosure?
3. Make sure the plugs are making good contact with the jacks.
4. Is the solder side of the PCB well isolated from the enclosure?
5. Check the stomp switch to make sure the appropriate terminals are connected/disconnected.
6. A cold solder joint where any wire connects to the board can cause an intermittent disconnect.  These can be hard to find, and it's sometimes easier to just reflow the joints.

If the problem is caused by a jack/pot/switch terminal contacting the enclosure, you can cure it by bending the terminals into a safer position, or wrapping them with electrical tape.

Sounds great! good job the mutron III is really an awesome and probably my fav envelope pedal! Im sure you'll find the loose connection. Build looks nice how did you do the graphics.

Thanks! I just printed them on clear Avery  "Easy Peel Labels" decals, then stuck them to the painted box and covered it with ~4 coats of Rustoleum clearcot. I've done it a few times, but I think I'm ready to find a different type of decal, because these always peel unless I put 6 coats or more, and the outline shows up too much.
