I did a quick glance, looks like the layout you show here somewhat corresponds with the schematic shown in the PDF here: http://www.madbeanpedals.com/projects/Deadringer/docs/DeadRinger.pdf
But I think some of the component designations are not the same. It would help if the whole picture was available, I can't tell exactly where ground is.

Can't help with any of the other questions though...
