Rehousing Blues Driver wiring SOS

I am in the process of rehousing my blues driver (for a second time), (the last show I played someone was trying top set their gear up behind ours while we were already setup and tripped on the input cord and screwed up my pedal ), I'm not sure what I have done wrong but the LED is now always on and the Momentary switch no longer switches the effect on and off, the switch now has to be held down, I have tried using a non-momentary and it seems to switch the effect on and off fine but there is still the problem of the LED and the fact that it works kinda scares me cause I know the pedal is supposed to use a momentary and did before the guy knocked it around.  Im fairly certain I have everything wired correctly as it was any idea what would cause this? If anyone could post a clear picture of a blues drivers guts I would really appreciate it all the ones I can find online seem to be covering all the parts I am second guessing myself on.
