Rokie here in need for some help, getting a pedal back together

Hello wonderful people,
I'm new to the site and am hoping for your help restoring my pedal to a playable state.
It's and old EHX Doctor Q Envelope Follower. I have very little experience in what connects where and reading schematics and transcode it to the actual board is beyond my current abilities. Ideally if you have the same pedal please post me a picture of how its all connected I can probably figure it out from there.

Welcome to the forum, by the way!    We can see from the schematic that the input jack goes to the 100k pot, so the output jack is the one unattached.  That input jack ground must go to....ground, too.

Output goes to the switch and ground.   The 'tip', or inner bent part of the jack, should go to the switch(es).   The 'barrel', or sleeve, of the jack, goes to ground.

Let us know if that's enough to get it running!   :)

I tried connecting everything as accurate as I could, but when I switch the pedal on I get constant hummmm. Where did I go wrong? Thank you for your help!

Are you sure the battery " - " has continuity with the board, and that each place that shows as grounded, IS grounded?  A loud "BZZZZZZZZZZZZZ" is often the sound of a missing ground.    If you have signal AND some buzz, but not as loud, that could be due to circuit not being in the enclosure, and maybe a fluorescent light/transformer nearby making noise...   

One possibility is that you have the jack signal wire(s)  (called the "tip" of the jack) reversed with ground (called "sleeve"...the sleeve is also the 'barrel' the plug is inserted thru). 

Also - maybe my seeing things wrong, but - left jack in the pic (must be input jack, it is TRS) - is that a SECOND wire on the tip terminal (with yellow wire), going to GND?   Where is that sucker headed?  Over to the switch?    As long as input is not grounded.....you might check that w/a  DMM set to continuity.

Do you have a DMM?    Looks like you are getting close now!

there is something wrong w/ the DC socket wiring. if no plug inserted, does that jack "short"? but, if plug is inserted, external DC will need to flow thru the battery. so, something wrong with the DC jack wiring.

Good eye Duck...yes.   For starters I'd eliminate the jack and see if the effect works without it.

@GibsonGM, that black wire (connected with yellow wire) going to the ground pin of the pot.

It looks to me like (in addition to the power jack issue), you need to sort out that input jack.   If I'm not mistaken (tired eyes....), the yellow wire may be attached to the "tip" of the jack, where your signal should be.    If it is grounded, it is just sending your signal to ground!   Tip should not be grounded...

The other 2 terminals are used to switch power on when you insert a plug (simply connects the ground to the barrel of the plug).   If you have a DMM, you can tell which terminal is the tip (farthest "notch" out....the one the plug tip connects to when inserted).

You'll sort it out! Just takes getting used to.  That jack doesn't look like the ones I use, and I can't see the 'wafers' from the side to know which is which, is all.
