Most low ESR's I see are quite tall.   When I repair stuff, the caps from smaller vendors tend to be even taller than the parts in equipment.    I have used tantalums in some charge pumps in the past.  For SMD you have the option of tantalums and (larger) ceramics.

FWIW, unlike yourself I haven't *had* to find very small low ESR caps.
