DOD 250 not working

I need some other eyes on this build...there doesn't seem to be any reason this isn't working. Getting bypass.

I'm not sure but it seems to me like IN-OUT-PCB-3PDT switch grounds aren't connected to PS ground (which is only connected to both pots lug 1..)

Ignore the above in case of indicator LED glowing when effect is ON..

Run a wire from your Output jack shield to the DC jack ground

SO many project pedals, whether boxed up or not, end up not working because of "wrong" jack lugs being connected.

One can begin troubleshooting by measuring the voltage on the V+ pin of the op-amp.

Under normal circumstances, you shouldn't get any response 'cause "this isn't working" says nothing..
