Following on from the thread about things that didn't work I finally built and verified my new layout for this.  And amazingly it works. 
I'd definitely consider adding a volume pot to it though, mine was very loud from the 100mV output.

I'm happy that you got it working. I think it was your original layout layout I used when I first build it. I ended up using point to point soldering.
