Do the switch grounds go to the board or to the in/out jack grounds?

One other thing, is "effect in" the board input or the lead from the input jack.

I think effect in and effect out are the board in and out.

The switch ground should go to the board ground. The jacks ground have to go to the board ground too, though that part of the circuit is not shown.

Luck!

Correct.

Just realized the issue.  Since the lugs are 3 X 3, orientation is EVERYTHING!  I wired it up with the lugs oriented vertical as in the drawing which was incorrect on my part.
