I have figured out all of the components except the following.  If you know what any of the following components are please post.

The brand and model number of Transformer (no number I can see)

The type of Diode in the left front corner (1n4) is on it and it seems to be glass.

There are 2 more diodes that look very similar to the front left one. on the very right in the pic.  has (16) on one side and a (4) i think on the top side of it.  It also appears to be glass.

the what i think is a rectifier on the dc jack.  has (4007 AF32) its black with a silver stripe and lettering on it.

The caps are as follows  (2A104J or 2A10UJ) cant tell for sure.  Made by tracon do you know what the value is and what type. POLY film?

(2A102JT) Tracon green too. do you know the value?

471J ceramic anyone know what brand?

Can anyone tell who makes the black electrolytic caps?

Any help you can give would be great.  Thanks

Yup, Octavia should cover it !!

Diodes : 1N4148 si
Transformer : Mouser 42TMO18/22 ( see octavia )

Large diode : 1N4007 ( 1N4001/4002 will be fine )

Caps : 104 = 100n ( 0.1uf ) poly/mylar
          10u = 10uf electro ( reticon/panasonic etc etc )
          102 = 1n ( 0.001uf ) poly/mylar
          471 = 470pf ceramic ( small brown )

Correct.  That is an already posted layout if I'm not mistaken.

I wonder whose layout it is, and which pedal it's from?  The layout is not from Tonepad, GGG, or Fuzz Central.

I'm just guessing based on a board sitting at home with a transformer sitting in the same corner of it.  Following CD's note, though, I took a look at the boards he notes and while those ones can also have the transformer sitting in the same corner, it does not have the same orientation.  So, my memory may be wrong here.  Perhaps it is NOT an already posted layout.

I also see four transistors - though one looks hidden in a shadow, so maybe there are three.  But that one in the lower right hand corner may be for a Millenium switch?
