The caps should be fairly easy to find. I don't think that the TL064 will be easy to find. www.mouser.com probably has those. You could also use a TL074 if you can't round up a TL064.

The IC, which is apparently a TL064, can be found at smallbear at http://www.smallbearelec.com/Detail.bok?no=197
As for C3, I can't read that schematic...
GILLES/Anybody: Do you know what C3 says there on that schematic??  "22m"???

It's 22n which is 0.022uF
