tonepad's Maestro fsh1; help please! LM13600 voltage readings

Hello

Could anyone let me know the voltages measured on the pins of the LM3600 in tonepad's maestro filter sample hold?  I had to draw my own layout to accomodate some large capacitors, and only realised a mistake after I'd plugged it in and smelt the burning.  I rectified the mistake with a jumper, and have measured all the power traces which are correct.  I'm now looking for which components I need to replace as I suspect at least one is dead.

           pin4    0.0V
           pin5    7.8V
           pin6   -7.2V
           pin7    0.0V
           pin8    0.0V
           pin9   -1.1V
           pin10  0.0V
           pin11  8.6V
           pin12  7.8V   
           pin13 -0.2V
           pin14  0.0V
           pin15  0.0V
           pin16 -6.0V
