Red Llama going crazy.

So I built up a Red Llama today and double checked all of my connections.  When I get to about 3/4 of the way up on the gain pot, the sound becomes really synth square-wave-fuzz moving into stepped oscillations. I used a CD4049UBE.  Are there any common causes of this type of problem in op-amp based circuits that I should look for?

Pardon if I only mention the obvious.  I believe un-used inputs must be connected to ground...or 9v....to avoid ocillations.
