Help with Millenium Bypass and amp footswitch indicators

I know, there are a lot of millenium bypass threads out there, but I couldn't find this one covered.  What I have is a three button footswitch where the buttons control boost, channel, and boost+channel running on a TRS cable by grounding relays in the amp.  I have the boost+channel switch set up so that if one of the other two buttons is activated, it is disabled.  I have LED indicators for each button and have them set up using the Millenium Bypass 2.  Everything for the boost and channel buttons works perfectly.  I had a problem with the boost+channel light turning off when the switch was deactivated, but fixed that.  The problem is that I did all of this without a cable connected.

So I hooked it up to my amp this morning and the boost and channel buttons work perfectly, no issues.  The problem is that the boost+channel LED is always on, regardless of whether the switch is activated or not.  I will try to get a wiring diagram posted when I get home this evening, but here is a description:

On the incoming TRS cable, sleeve is ground, ring is boost and tip is channel.  The boost and channel buttons are 3PDT's and the boost+channel button is a DPDT.  The boost+channel button has boost running to one center lug and channel to the other center lug.  When that switch is activated it sends them out to the boost switch where two sets of lugs are used to break the signal if boost is activated.  Then they run from there to channel and do the same thing.  This means that when boost or channel is activated, boost+channel can't make the connection to ground, regardless of whether its switch is activated or not.  The issue is that somehow the millenium bypass is seeing something that lets the LED stay on all the time.  Does anyone have any idea what could be causing this?  I have tried a few different things, but haven't gotten it to work.  Like I said, a diagram is coming.  Thanks.
