I'm trying to understand why sometimes buffers are okay for sending signal to tuners and why in other circumstances an isolation transformer is needed.
What are the conditions that change this?

The conditions which change this are whether the tuner needs ground isolation from the signal circuits or not. It's a power and signal ground issue.

Thanks for the reply. I understand that the transformer isolates devices from each other by their ground. I was just wondering how it is that some musicians do fine with the buffered tuner out while others also necessitate an iso transformer.

It's probably that their amps leak a little AC into signal ground, or that the venue radiates a lot of 60 Hz hum. I think it has little to do with the tuner and much to do with the rest of the setup. Just my suspicion.
